THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our commitment to your privacy
Loudoun Home Care LLC is required by law to maintain the privacy of your protected health information (“PHI”), to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect. PHI is information about you, including demographic information, that may identify you and that relates to your past, present, or future physical or mental health and the care or payment for that care.
How we may use and disclose your health information
For treatment
We may use and disclose your PHI to provide, coordinate, and manage your care — for example, sharing information among our nurses, caregivers, your physicians, pharmacies, and other providers involved in your care.
For payment
We may use and disclose your PHI to obtain payment for the services we provide — for example, to bill and collect from you, an insurer, or another responsible party, including verifying coverage and benefits.
For health care operations
We may use and disclose your PHI to run our agency — for example, quality assessment, caregiver training and evaluation, scheduling, licensing, and general administrative activities.
Other uses and disclosures permitted or required by law
We may use or disclose your PHI without your authorization in certain situations, including: as required by law; for public health activities; to report abuse, neglect, or domestic violence; for health oversight activities; in judicial and administrative proceedings; for law enforcement purposes; to coroners, medical examiners, and funeral directors; for organ and tissue donation; for approved research; to avert a serious threat to health or safety; for specialized government functions; and for workers’ compensation as authorized by law.
Uses and disclosures that require your authorization
Most uses and disclosures of psychotherapy notes, uses and disclosures for marketing purposes, and any sale of PHI require your written authorization. We will also obtain your written authorization for other uses and disclosures not described in this Notice. You may revoke an authorization in writing at any time, except to the extent that we have already taken action in reliance on it.
Your rights regarding your health information
- Right to inspect and copy your PHI in our records, subject to limited exceptions;
- Right to request an amendment if you believe your PHI is incorrect or incomplete;
- Right to an accounting of certain disclosures we have made of your PHI;
- Right to request restrictions on how we use or disclose your PHI;
- Right to request confidential communications by alternative means or at an alternative location;
- Right to a paper copy of this Notice, even if you agreed to receive it electronically; and
- Right to be notified in the event of a breach of your unsecured PHI.

Our responsibilities
We are required to maintain the privacy of your PHI, provide you with this Notice, abide by its terms, notify you if a breach affects your unsecured PHI, and not use or disclose your information except as described here or as otherwise permitted by law.
Changes to this Notice
We reserve the right to change this Notice and to make the revised Notice effective for the PHI we already have, as well as information we receive in the future. The current Notice will be posted on this website and available at our office, with its effective date shown above.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.
